Airway hygiene is a two-system job
Mucociliary clearance provides continuous housekeeping in the conducting airways. The cough reflex is the rapid-flow, high-shear backup that clears what mucociliary clearance can't — especially from the larger, central airways.
When coughing becomes difficult
Muscle weakness or loss of diaphragm control can make the cough reflex ineffective. When that happens, your care team may prescribe manual or mechanical assisted cough therapy.
Mechanical assist (MI-E): a cough assist device gently blows air into the lungs, then quickly pulls it out. One "in-and-out" is a cough cycle; a typical therapy uses 4–6 cycles in a row.
Assisted cough therapy can make breathing easier and help patients feel more comfortable day to day.

The assisted cough cycle
A cough cycle is pause, inhale, exhale — delivered in sequences of 3–6 cycles, with rest between sets. Your care team sets the therapy to your comfort.

Decades of published MI-E literature
Mechanical insufflation-exsufflation has been studied and recommended in clinical literature for years — including long-term use in neuromuscular disease and home MI-E use. Selected publications include Homnick (Respir Care 2007), Chatwin & Simonds (Respir Care 2020), and Moran et al. (J Paediatr Child Health 2013).

What does therapy feel like?
Most people describe it as a deep breath in, then a strong "pull" out — like a cough the device does with you. Settings are adjusted to your comfort, and CoughCue can synchronize therapy with your own breathing effort.
How often do I do therapy?
Your prescriber sets your therapy plan. A typical session uses sets of 3–6 cough cycles with short rests, repeated until secretions are cleared — one or more times a day as prescribed.
Can I travel with it?
Yes — the system is 9 lbs with a built-in battery (up to 2 hours of therapy on a new, fully charged battery) and packs into its padded carrying bag.
What is Arc Connect?
A free mobile app that helps you track therapy, set goals and reminders, and share progress with your care team when you choose. With compatible BiWaze Cough systems, it can also show live therapy waveforms.
Is BiWaze Cough covered by insurance?
BiWaze Cough has established coding — HCPCS E0482 for the device and A7020 for the breathing circuit — but coverage varies by payer and plan. Your equipment provider and care team can confirm your specific benefits — and ABM Respiratory Care provides documentation and appeal support to help.
